A group of specialists at the University Hospital of Navarra has shown that it is possible repigment chronic forms, localized and stable vitiligo, a degenerative disease of the skin where the cells responsible for pigmentation, melanocytes die and stop producing melanin.

They have done by transplanting the patient’s own epidermal cells of a pigmented area on certain areas which appear depigmented. “The novelty of this method is that we grow epidermal cells in monolayer on a substrate of amniotic membrane,” explains Pedro Redondo, Pamplona center of that dermatologist and director of the work whose results were published in the official magazine of the British Association of Dermatologists. As noted, his team has treated ten patients with stable vitiligo by transplantation of these cells, and all have achieved high success rates, “witha repigmentation of 75 to 100 percent of the affected area.

As stated by Felipe Prosper, area director of the Clinical Cell Therapy, the treatment of vitiligo by this new technique begins with the cultivation of epidermal cells and keratinocyte-demelanocitos mixture, to then proceed to make selective crops. “To date there were no jobs in monolayer cultures of melanocytes because they had not found adequate support to achieve them,” says Prosper. In addition, Redondo stresses that “the best indication is the treatment of targeted areas, who have not responded to conventional medical treatments.

A wide variety of substances can cause a skin condition called contact dermatitis, which is often controlled with a proper diagnosis and treatment, experts say.

Contact dermatitis is a chronic or acute inflammation of the skin resulting from contact with a chemical, biological, or physical. The most common culprits include cosmetics, perfumes, certain foods, nickel and other metals, cleaning solutions and detergents, industrial chemicals and rubber latex.

The majority (between 20 and 35 per cent) of cases of dermatitis affecting the hands, including over 75 percent of cases of contact dermatitis related to the workplace, according to experts at the annual meeting of the American College of Allergy, Asthma & Immunology (ACAAI) in Seattle.

“Allergic contact dermatitis of the hands is very common in certain trades. As such health professionals, hairdressers and cosmetologists, carpenters, machine operators or those working with chemicals,” said Dr. James. S. Taylor, director of the Section of industrial dermatology at the Cleveland Clinic in Ohio, in a press release from the ACAAI.

Patch tests are important in the diagnosis of hand dermatitis, as allergic contact dermatitis may appear alone or with irritant dermatitis, atypical or otherwise, Taylor said. Treatment depends on the cause of dermatitis.

“To relieve the symptoms of allergic contact dermatitis, it is necessary to omit or substitute or use the allergen usually topical or systemic corticosteroids occasionally,” said Taylor.

The second most common site for dermatitis is the face and eyelids, often due to a reaction to a cosmetic, soap or other personal care product.

“The patient has to change to a new brand of makeup or a different cleaner before you see the skin rash. Normally, we treat this condition with a topical corticosteroid and we tell the patient to stop using the product,” stated Dr. Vincent A. DeLeo, chief of dermatology at Beth Israel Medical Center and St. Luke’s-Roosevelt in New York City, in the press release of the ACAAI.

Usually the patch test is not required in cases of dermatitis of the face and eyelids, but may be required if the patient does not improve after switching products skin care and topical treatment, said DeLeo.

Ultrasound is a therapeutic modality that has been used by physical therapists since the 1940s. Ultrasound is applied using a round-headed probe or tube is placed in direct contact with the patient’s skin. Ultrasound gel is used on all surfaces of the head, in order to reduce friction and assist in the transmission of ultrasound waves. Therapeutic ultrasound is in the frequency range of about 0.8-1.0 MHz

The waves are generated by a piezoelectric effect caused by the vibration of crystals in the head of the wand / probe. The sound waves that pass through the skin cause a vibration of the tissues. This vibration or cavitation can cause a deep heating locally though usually no sensation of heat will be felt by the patient. In cases where a heating effect is not desirable, as a new injury with acute inflammation, the ultrasound can be pulsed continuously instead of transmission.

Ultrasound can produce many effects other than just the potential heating effect. It has shown it can cause increases in tissue relaxation, local blood flow, and scar tissue breakdown. The effect of increased local blood flow can be used to help reduce local swelling and chronic inflammation, and according to some studies, promote bone fracture healing. The intensity or power density of ultrasound can be adjusted depending on the desired effect. A higher power density (measured in watt/cm2 is often used in cases where scar tissue breakdown is the goal.

Ultrasound can be used to achieve phonophoresis. This is not an invasive form of drug delivery to the tissues beneath the skin, perfect for patients who are uncomfortable with injections. With this technique, the ultrasonic energy forces the medication through the skin. Cortisone, used to reduce inflammation, is one of the most commonly used substances delivered in this way.

A typical ultrasound treatment will take 3-5 minutes. In cases where scar tissue breakdown is the goal, this time of treatment may be much longer. During the treatment the head of the ultrasound probe is kept in constant motion. If it is kept in constant motion, the patient should feel no pain at all. If the probe is carried out in one place for more than a few seconds, an accumulation of sound energy that can produce can become uncomfortable. Interestingly, if there is even a very small break in a bone in the area that is near the surface, a sharp pain can be felt. This occurs as the sound waves get trapped between the two sides of the break and build up to become painful. Thus, ultrasound can often be used as a fairly accurate tool for diagnosing minor fractures that may not be evident on the radiograph.

Some conditions treated with ultrasound include tendonitis (or tendinitis if you prefer), no sharp pain, muscle spasms, and even Peyronie’s Disease (to break up scar tissue). Contraindications of ultrasound include local malignancy, metal implants below the area treated, local acute infection, vascular abnormalities, and directly on the abdomen of pregnant women. It is also contraindicated to apply ultrasound directly over active epiphysis regions (growth plates) in children, along the spinal cord in the area of a laminectomy, or over the eyes, skull, or testes.
